Amalgam fillings are those that are made up of 50% mercury and the other 50% is a compound of different metals like silver, copper, zinc and tin.
The important factor to understand about amalgam fillings is that they corrode easily, especially in a humid environment like the mouth and when in contact with other metals from other amalgam fillings, old crowns, implants, bridges, metal prosthetics, etc. creating galvanic forces, known as the battery effect.
Mercury and its toxicity
The mercury absorbed by the body takes over the normal cell activity acting as a hormonal disruptor, this can translate in different illnesses, disorders and problems such as burn out, irritability, headaches, dizziness, shaking, irritable bowel syndrome, memory loss, coordination problems, skin issues, allergies, depression, etc.
Safe removal protocol
Taking into account that mercury vapour is mostly released when an amalgam is placed and extracted it is very important to follow a safe removal protocol.
The protocol recognised by the IAOMT consists of:
- Use of PPE (personal protective equipment) by the dentist and dental nurses, to avoid inhaling mercury vapour during the treatment.
- Physical barrier (rubber dam) on the patient’s mouth to avoid ingesting part of the amalgam filling.
- Avoid overheating the amalgam by use of cooling water when drilling.
- Cut the amalgam with special burs.
- High-volume air filtration system (such as an at source oral aerosol vacuum) capable of removing mercury vapor and amalgam particles generated during the removal of one or more mercury fillings.
- Allow for ventilation in the surgery.
- Detox the body with chelating agents.
